PARTICIPANT WAIVER / ONLINE MEDICAL SCREENING: All participants are required to complete a Participant Agreement prior to taking part in an on-ice session. An online medical screening questionnaire must be completed each time your player is on the ice with HSC.
PAYMENT: We are currently accepting payment via electronic transfer only.
CANCELLATIONS / REFUNDS: There will be no deductions or refunds for arriving late, leaving early, or missing part of a session. Exceptions may be granted for illness or medical reasons if we are notified at least 24 hours in advance. If we are able to fill the spot, we will refund the session fee minus a 25% administrative fee. If we are forced to cancel sessions due to facility closures, refunds will be issued immediately.
MISCELLANEOUS: HSC Hockey School does not provide any equipment. It is the participant’s responsibility to ensure they have all the necessary equipment to play ice hockey and that the equipment is in good (certified) playing condition. HSC Hockey School adheres to Hockey Canada, USA Hockey and IIHF guidelines. HSC Hockey School will not accept responsibility for misplaced, lost or stolen equipment of personal belongings. All equipment and personal belongings should be clearly marked.
PARENT’S / GUARDIAN’S RELEASE: HSC Hockey School reserves the right to dismiss a participant when it is determined to be in the best interest of the child and/or the camp. In the event that a participant is dismissed from the camp, there will be NO REFUND of fees. HSC Hockey School will provide necessary and appropriate supervision of all participants. By signing below, I give my approval for my child to participate in all activities and assume all risks and hazards incidental to such participation and do waive release, absolve, indemnify and agree to hold harmless, other than for willful default or neglect on their part, HSC Hockey School and its staff. I certify that my child is in good physical and mental health. I understand that participants are assigned to teams/groups on the basis of their age, size, and ability to ensure fair competition for all. Every effort will be made to honour teammate requests; however, HSC Hockey School reserves the right to assign participants as determined to be in the best interest of the program and the individual.
